Assistive Technology Assessments
An Assistive Technology (AT) Assessment identifies the most appropriate equipment and technology to support a person’s independence, safety, and participation in everyday activities. At Independence Pathways OT, AT assessments involve a detailed evaluation of functional needs, environments, and current supports, with consideration of both basic and complex solutions. Recommendations are practical and evidence-based, ensuring equipment is fit-for-purpose, cost-effective, and aligned with NDIS requirements, while maximising independence and reducing reliance on ongoing supports.
Home Modification Assessments
A Home Modifications Assessment evaluates how a person’s home environment supports or limits their safety and independence in daily activities. Recommendations range from minor modifications to complex structural changes, all tailored to the individual and aligned with NDIS requirements. Home and Living Assessments
A Home and Living Assessment evaluates a person’s current living situation and identifies the supports required to live safely, independently, and sustainably. At Independence Pathways OT, this includes comprehensive assessment of functional capacity, daily support needs, and environmental requirements to inform recommendations for Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) where appropriate. Symptom Management
Symptom Management focuses on supporting individuals to manage the impact of fatigue, cognitive changes, pain, and physical conditions such as arthritis on daily function. At Independence Pathways OT, this involves practical, individualised strategies including energy conservation, task modification, cognitive supports, and environmental adaptations. The goal is to optimise function, reduce the impact of symptoms on everyday activities, and support safe, sustainable independence across home and community environments.
Care Plans and Training
Care Planning and Carer Training focuses on developing practical, individualised support strategies that align with a person’s functional needs, goals, and daily routines. At Independence Pathways OT, this includes creating clear, evidence-based care plans and providing hands-on training for carers to ensure supports are delivered safely, consistently, and effectively. Training may cover manual handling, use of assistive technology, communication strategies, and task-specific support, with the aim of improving outcomes, reducing risk, and promoting greater independence.
