Home Modification Assessments
At Independence Pathways OT, Home Modifications Assessments are completed to improve safety, accessibility, and independence within the home environment.
Assessments focus on identifying barriers that impact a person’s ability to safely access and use their home, and developing practical modification recommendations that support both current and future functional needs.
Recommendations are tailored to the participant’s physical, cognitive, sensory, and functional presentation, as well as the layout and structural limitations of the home.
What are Home Modifications?
Home Modifications are structural changes made to a home to improve accessibility, safety, and functional independence.
Modifications may include:
Grab rails and handrails
Ramps and step modifications
Bathroom modifications, hobless or accessible showers
Toilet modifications
Widened doorways
Access pathways
Kitchen modifications
Recommendations may range from minor modifications through to complex home modifications depending on individual needs.
What is included in a Home Modifications Assessment?
Assessments may include:
Functional assessment within the home environment
Assessment of mobility, transfers, and daily living activities
Identification of safety risks and accessibility barriers
Measurement of relevant spaces and access areas
Consideration of current and anticipated future needs
Consultation with participants, families, carers, and support teams
Liaison with builders, suppliers, and other professionals where required
Development of practical modification recommendations
Comprehensive clinical reporting aligned with NDIS requirements

supporting documentation for NDIS funding applications
coordination with builders or project teams
review of plans or proposed modifications
