Home and Living Assessments

At Independence Pathways OT, Home and Living Assessments are completed to identify the supports, environment, and accommodation arrangements required to help participants live as safely and independently as possible.

Assessments consider the participant’s functional capacity, daily support needs, living environment, informal supports, and long-term goals to determine the most appropriate housing and support options.

This may include assessment and recommendations relating to Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Individualised Living Options (ILO), supported accommodation transitions, capacity building for independent living and in-home support requirements.

 

What is included in a Home and Living Assessment?

Assessments may include:

  • Review of relevant medical and allied health documentation

  • Functional assessment of daily living skills and support needs

  • Assessment of mobility, transfers, cognition, behaviour, and safety

  • Review of current living arrangements and support systems

  • Identification of barriers impacting independence and community participation

  • Assessment of risks within the home environment

  • Consultation with participants, families, carers, and support teams

  • Consideration of future needs and sustainability of supports

  • Comprehensive reporting aligned with NDIS Home and Living requirements

Reports are completed with consideration of the participant’s ability to safely and effectively complete daily tasks both with and without support.

 

Supported Independent Living (SIL)

SIL Assessments consider the level and type of support a participant requires within the home environment to complete daily activities safely and consistently.

This may include assessment of:

  • support hours and staffing ratios

  • overnight support requirements

  • prompting versus physical assistance

  • supervision and risk management

  • support required across a 24-hour period

Recommendations are based on observed functional impact and the participant’s demonstrated support needs.

 

Specialist Disability Accommodation (SDA)

SDA Assessments consider whether a participant’s functional impairments and support needs indicate a requirement for specialised housing solutions.

This may include consideration of:

  • accessibility requirements

  • wheelchair access and mobility needs

  • environmental supports

  • high physical support needs

  • robust or improved liveability design categories

  • long-term housing suitability

Assessments are completed in line with NDIS SDA evidence requirements and consider both current and anticipated future needs where relevant.

Who May Benefit from a Home and Living Assessment?

Home and Living Assessments may benefit people who:

  • are unable to safely live independently without support

  • require high levels of daily assistance

  • are transitioning between living environments

  • require evidence for SIL or SDA funding

  • have progressive neurological or complex physical conditions

  • require assessment of overnight support needs

  • are at risk within their current living environment

  • require increased structure or support to maintain tenancy and daily functioning
